The actual service is called AllClear ID Plus, and provides alerts, insurance coverage, and access to fraud investigators. If you're a PlayStation Network or Qriocity account holder with an active account as of April 20, 2011, you're eligible for twelve months of the program. To sign up, head over to Sony's Identity Theft Protection page. Sign in with the e-mail address associated with your PSN or Qriocity account, and you'll be sent an activation code for AllClear ID Plus.
Sony has maintained that it hasn't received any reports of fraudulent activity as a result of thedata breach, but it's better to be safe than sorry. Taking the company up on a free offer might save you a headache, and can't really hurt in any event.
